On this week's episode Liz and Jeff speak with Dr. Kirsty Bannister, a renowned researcher on conditioned pain modulation, about what we know, what we’re studying, and what we can look forward to in the future of this topic. Kirsty completed BSc, MRes and PhD degrees at University College London and and Imperial College before returning to UCL for a decade-long postdoctoral research position in the laboratory of Professor Anthony Dickenson. In 2017 Kirsty gained her faculty position upon joining King’s College London where she now runs her research group investigating the central modulation of pain. Specifically Kirsty's research team bridges the gap between bench and bedside pain research by conducting exploratory experiments that seek to molecularly, anatomically and/or functionally define descending control pathways in rodent and man using electrophysiology and human psychophysics respectively. In this episode, Liz and Jeff chat with Walt Fritz. Walt has been a physical therapist since 1985 and has integrated manual therapy into that model ever since 1992. He trained with many traditional tissue-based instructors, including myofascial release and craniosacral therapy, but became dissatisfied with the narratives used to describe the work and effects and began to move into a more biologically plausible explanation. While continuing to use a hands-on approach that visibly resembles those earlier models, he has neutralized the implied message by now referring to the work he uses and teaches as "manual therapy." Building on concepts firmly grounded in the biopsychosocial model, as well as the possibly undervalued third leg of the evidence-based practice model (patient preferences and values), his model of intervention and professional education stresses a patient-led experience. He believes that manual therapy need not be a passive modality, one that fosters patient dependency, but a subset of therapeutic intervention that reduces fear of movement and encourages self-efficacy, one well-grounded in evidence-based practice. Dr. Justin Dunaway joins Pain Reframed this week! Justin has extensive experience in outpatient orthopedics, not only in the United States, but also in Haiti. Justin and his wife, Dr. Morgan Denny, founded and run the organization STAND Haiti. The STAND Haiti Project establishes permanent access to orthopedic rehabilitative services in the country of Haiti through direct patient care and clinical training of its citizens. Through STAND Haiti, Justin and his team go to Haiti multiple times a year to see patients who have a significantly different relationship with healthcare and different level of resources and education than in the United States. Interestingly, the difference of access to resources and education brings up some fascinating discussion. Jeff, Justin, and Tim share their thoughts on these differences and how having a “blank slate” in treatment and care can actually be beneficial. Justin shares his experience in going through the Therapeutic Pain Specialist Certification program and how it helped him and changed the way he manages pain. The certification was instrumental in teaching Justin ways to take the ideas on pain management and apply it in a framework that works for him and his patients.
